What Is a Breast Lift?

A breast lift (mastopexy) is a surgical procedure that reshapes and raises the breast by removing excess skin and repositioning the nipple-areola complex to a higher position on the breast mound. It does not add volume — it reshapes existing volume. Mastopexy addresses ptosis (breast sagging), which is classified in grades from Grade 1 (mild, nipple at the level of the breast fold) through Grade 3 (severe, nipple significantly below the breast fold). The appropriate technique depends on the degree of ptosis and the amount of excess skin.

Who Needs a Lift?

Candidates for mastopexy typically include: women with significant post-pregnancy or post-weight-loss ptosis; women with naturally developed significant ptosis; women after explant (implant removal) where the skin has stretched to accommodate implants and does not retract fully; and women seeking to raise and reshape breasts that have sagged due to aging and gravity. Mastopexy is typically recommended when augmentation alone would not achieve the desired result because the nipple position is low or the breast envelope is excessive.

Lift with Augmentation

Mastopexy-augmentation (lift combined with implants) is one of the most technically complex procedures in plastic surgery. It addresses both volume (via the implant) and shape/position (via the lift) simultaneously. The technical complexity arises from the competing goals: the implant needs space and skin expansion while the lift is removing excess skin. Many surgeons prefer to stage the procedures (lift first, augmentation later) for safer, more predictable results, though combination procedures are performed and have good outcomes in experienced hands.

Does a breast lift leave scars?

Yes — mastopexy involves skin removal and always leaves scars. The extent of scarring depends on the technique used: a periareolar lift leaves a scar around the areola only; a lollipop lift adds a vertical scar from the areola to the breast fold; a full anchor/inverted-T lift adds a horizontal scar along the breast fold.
